Psilocybin for Migraine: What Early Research Shows

Can psilocybin prevent migraine? What the first controlled studies found, why a low dose may matter, and why the evidence is still very early.

Migraine affects around one in seven people worldwide and is one of the leading causes of disability, especially in younger adults. Many people find that current treatments only partly help. As a result, researchers have started to test new ideas, including psilocybin for migraine. This article explains what the first controlled studies found and why the evidence is still at a very early stage.

This article is for general information only. It is not medical advice. If you live with migraine, please speak to a doctor or headache specialist about the treatments available to you.

Why Researchers Are Interested in Psilocybin for Migraine

Serotonin plays an important role in migraine, and several migraine medicines act on serotonin receptors. Psilocybin also acts mainly on serotonin receptors, especially the 5-HT2A receptor. In addition, people with cluster headache, a different and very painful headache disorder, have long reported that psilocybin helps them. Our article on psilocybin for cluster headaches covers that research. These reports led scientists to ask whether psilocybin might also help migraine.

The First Controlled Study of Psilocybin for Migraine

In 2021, researchers at Yale University published the first controlled study of psilocybin for migraine [1]. Ten adults with migraine took part in two sessions: once they received a low dose of psilocybin, and once a placebo. Neither the participants nor the researchers knew which was which.

In the two weeks after psilocybin, participants had fewer migraine days than after the placebo. Notably, this benefit did not depend on how strong the psychedelic effects felt during the session. Participants tolerated psilocybin well, and there were no serious side effects. The researchers concluded that a single dose might have a lasting effect on migraine, which differs from how most migraine medicines work.

A Follow-Up Study With Repeated Doses

In 2025, the same research group published a second small study with 18 adults [2]. Participants received zero, one or two doses of psilocybin a week apart, and an antihistamine as an active placebo at the other sessions. The researchers used this active placebo to make it harder for people to guess what they had taken.

The results were mixed. People who received psilocybin had large reductions in migraine days in the following two weeks. However, the differences between groups were not statistically significant, partly because the study was so small. In addition, all groups, including the placebo group, improved over the following eight weeks. This shows how important placebo effects can be in migraine research.

What This Means for Psilocybin and Migraine

These early studies are encouraging, but they are far from proof. Both trials were very small, and larger trials are needed to show whether psilocybin really prevents migraine and for how long. Until then, psilocybin for migraine remains experimental and is only available in research studies.

Self-treatment also carries risks. Psilocybin can cause anxiety, confusion and a temporary rise in blood pressure. Some migraine medicines, such as triptans, also act on serotonin receptors, and researchers have not studied interactions well. Read our article on psilocybin and antidepressants to learn how other serotonin medicines can interact with psilocybin.

Who Should Be Careful

Clinical trials usually exclude people with heart disease, uncontrolled high blood pressure, or a personal or family history of psychosis or bipolar disorder. Because migraine with aura is linked to a slightly higher risk of stroke, heart and blood vessel health also matters. Our article on psilocybin and the heart explains these effects.

Frequently Asked Questions

Is psilocybin approved for migraine?

No. Researchers are only testing psilocybin for migraine in studies so far, and no country has approved it as a migraine treatment.

Can microdosing help migraine?

There is no reliable evidence for this. The studies so far used a single low dose or two doses, not regular microdoses. See our microdosing guide for more.

Can psilocybin trigger headaches?

Yes. Headache is one of the most common side effects of psilocybin, and it usually passes within a day.

Conclusion: Psilocybin for Migraine Needs More Research

Early studies suggest that a low dose of psilocybin might reduce migraine days for a while, even without strong psychedelic effects. However, the research is small, the results are mixed, and placebo effects are large. For now, proven migraine treatments remain the best option.
